Enrollment Process

CW offers social learning, emotional regulation, sensory integration, occupational therapy and traditional speech and language therapy sessions for children aged 3-18 years old and adults. Group and individual sessions are available depending on the needs of the client. If you would like to initiate the enrollment process, please begin by completing:

STEP 1:

There are two options for completing and intake form. You can fill out the form digitally or manually.

The preferred option is to download and fill out our digital form and simply email it to us. To do this, click here to download the PDF form to your computer. Open and complete the form, save your changes and email the completed form to intake@cwtherapy.com.

If you prefer the paper method, click here to download the PDF, print and fill out by hand then mail or fax the intake form to CW.

The intake form will give the therapist(s) a good background on your child and the concerns you wish to address so that we are prepared for the consultation.

STEP 2:

You will be contacted either by email or phone within 2-4 business to schedule an initial consultation to gather more information on an appropriate placement for you or your child.. If there are no spaces available, you will be notified and your child will be put on our waiting list. CW will contact you when a space is available.

We require this initial consultation for all new students in order to determine the correct therapy and or placement in a group. It is not a formal evaluation or assessment and should take 20-30 minutes.

Please mail or fax a copy of the below reports to CW at least seven days prior to your scheduled consultation.

  1. Your latest IEP or school evaluations (if applicable)
  2. Any outside therapy reports (OT, PT, SLP)
  3. Any diagnostic reports or updates

The fee for the initial consultation is $60.00 which will be applied to any future services with Communication Works.

STEP 3:

After the consultation, you will be notified of the therapists' recommendations for services. If you agree to continue, we will attempt to place your child or yourself in an appropriate group or schedule the appropriate therapy. Depending on the therapy, there may be a deposit due at that time. Clients are placed in groups with peers who are matched for cognitive, behavioral and social needs. A good deal of time is invested in considering each client's placement in a group.

Continuing Clients:

If you or your child has been a client of Communication Works previously and you wish to register for the upcoming session, please complete a Registration Form and mail it to the office with your registration fee. Clients are placed in groups with peers who are matched for cognitive, behavioral and social needs.  If we cannot find an appropriate group, your registration fee will be fully refunded.  If a group placement is identified, your registration fee will go toward the first two upcoming sessions.
