Enrollment Process
CW offers SLG sessions for children aged 3-18 years old and adults. If you would like to initiate the enrollment process, please begin by completing Step 1 below.
STEP 1:
Complete online or download and mail or fax the intake form for your child or the adult intake for yourself.
STEP 2:
You will be contacted either by email or phone informing you that we have received your application. If a space is open, CW will be set up a pre-group interview/ consultation to gather more information on an appropriate placement for you or your child.. If there are no spaces available, you or your child will be put on our waiting list and you will be contacted when a space is available.
We require this pre-group interview/consultation for all new students in order to determine the correct placement in a group. It is not a formal evaluation. We will set up a time for you or your child to come in for this consultation (20-30 minutes depending on the client's age). The fee for the process is $60.00 which is non-refundable and is not applied to the group cost.
STEP 3:
Once your consultation is scheduled, please fill out the Parent Checklist and Parent Questionnaire forms. (Adult clients can skip to step 4)
Along with the completed forms, we ask parents to include a recent copy of their child’s IEP or 504 plan and any medical or diagnostic reports (within the last year) that are relevant to the student’s academic, behavior and language abilities. Please send any copies to CW at least seven days prior to their scheduled consultation.
STEP 4:
If we determine that you or your child will benefit from our services, we will attempt to
place him or her in an appropriate group. At this point, please complete a
Registration Form and submit a $150 registration fee. We form groups
according to clients'’ needs, ages, and grade levels. If we cannot find an
appropriate group, your $150 registration fee will be fully
refunded. If a group placement is identified, your registration fee will go
toward the first two upcoming sessions.
