New Clients

STEP 1 Complete Enrollment Form
There are two options for completing an enrollment form. You can fill out the form online (digitally) or by hand (manually).

To Submit Online (digitally)

  1. Enter the information requested directly into the fields of this form-link found below.
  2. Save the completed form to your computer.
  3. Email the completed form (as an attachment) to enrollment@cwtherapy.com.

To Submit by Mail (manually)

  3. Mail or fax (510-639-2977) the enrollment form to CW.

STEP 2 Wait For Our Call

You will be contacted either by email or phone within 2–4 business days. If we have a possible opening in a group, we will schedule an informal consultation (meet-n-greet) if it is deemed necessary to gather more information for an appropriate placement. Payment of $90 is due prior to or at the end of this meeting. If there are no spaces available, you will be notified and put on our waiting list. CW will contact you when a space becomes available.

Please mail or fax a copy of all available reports to CW at least 5 days prior to your scheduled meeting, such as:

  1. Any outside therapy reports (psychological, speech and language, etc.)
  2. Any diagnostic reports or updates

STEP 3  Group Placement
Following the informal consultation, you will be notified of CW’s recommendation for services. If you agree to continue, we will gather your availability and schedule and attempt to place the applicant in an appropriate group and/or individual therapy. To help with the labor intensive process of scheduling groups we encourage you to provide as many times and days as possible to allow for the most options. This will increase the chances of finding an optimal placement. Clients are placed in groups with peers who are matched for cognitive, behavioral, and social needs. We spend a substantial amount of time trying to identify the best group placement through our informal consultation (meet-n-greet) and comprehensive questionnaires; however, sometimes it is not until we spend time with the client in the group over several sessions that we can feel confident about the placement. We typically allow a 3-week trial period to ensure proper placement and to make changes if necessary. Additionally, we assess each group member’s placement throughout the year to make sure clients are in a group that is meeting their needs and not moving too fast or too slowly for their social development.

For some clients, individual services are recommended.  This can help provide the foundational skills to learn some of the core vocabulary and concepts before jumping in with peers.  This can also be helpful for clients that have specific needs or areas to address that might differ from those in a group setting and best to address individually.
